An experimental study of hypercholesterolemia-induced disorder of Oddi's sphincter

[Abstract]AIM:To investigate the functional changes and histopathological basis of Oddi's sphincter under hypercholesterolemia.METHODS:Twenty-four New Zealand rabbits were equally divided into three groups randomly. Experimental groups I and II fed with cholesterol-added forage for 4 and 8 weeks respectively before sacrificed. Images, functions and histopathological characteristics of Oddi's sphincters of experimental groups were studied and compared with control group by cholangiography, catheter manometry, and quantitative analysis of nitric oxide synthase.RESULTS:The basal pressures of proximal low-pressure segment of Oddi's sphincter of both experimental groups I and II (20.9±6.1 mmHg, 25.6±9.1 mmHg, respectively) were higher than those of control group significantly (11.7±2.8 mmHg, P<0.01, P<0.01). The basal pressures of distal high-pressure “valve” segment of both experimental groups (138.4±45.5 mmHg and 144.5±40.4 mmHg, respectively) were significantly higher than those of control group (69.3±9.8 mmHg, P<0.01 and P<0.01). The gallbladder volumes of experimental group I and II (3.4±1.0 mL and 3.9±1.9 mL) were significantly larger than those of control group (2.0±1.9 mL, P<0.05 and P< 0.01). Between groups there were no significant differences in nitric oxide synthase contents. Enlargement of the cell volume, wavy distortion of the cell membrane, loosening and disarrangement of myofilament, uneven in size and distribution of dense body, angulation of longitudinal axis between dense body and myofilament, swelling, congregated at one end of nucleolus and disappearance of cristae of plasmosomes and concentration of chromosome at nucleolus margin were observed in experimental groups under electron-microscope.CONCLUSION: The diastole function of Oddi's sphincter could be damaged by hypercholesterolemia, inducing a functional disorder of Oddi's sphincter with increased resistance in bile excretion. The result is retention of bile in the gallbladder.

胆管Oddi括约肌功能紊乱(sphincter of Oddi dysfunction, SOD)的报告始见于胆囊切除术后的病人。由于病因不明,在排除炎症、结石、狭窄等器质性病变后,又将其称为胆囊术后综合征。随着经内镜逆行插管测压技术的应用,目前认为SOD最重要的改变为胆管Oddi括约肌(sphincter of Oddi, SO)基础压升高,并将其做为SOD诊断的金标准[1]。根据进一步研究,有作者[1、2]推测SOD不仅仅发生于胆囊切除术后的病人,认为在胆囊切除前可能就已经存在SOD。虽然1997年Szilvassy等[3]发现并首先报道临床证实的1例高胆固醇及高甘油三脂血症伴有SO功能异常的病例,但到目前为止尚无动物模型及组织形态学观察证实这一观点。
胆囊胆汁淤积是胆囊结石形成的重要先决条件。根据解剖结构特征,胆管SO是胆囊及肝胆汁排泄的唯一通道,显然胆囊胆汁淤积与胆管SO功能状态有着直接关系。为了探讨在胆囊胆固醇结石形成前胆管SO的运动功能特征,我们用兔复制高胆固醇血症模型,观察高胆固醇血症作用下胆管SO影像功能变化及组织形态改变与胆囊淤胆间的关系。
材料和方法
一、动物模型复制及判断标准
纯种新西兰兔24只,3~4个月龄,体重2.0~2.5 kg,购自本校动物实验中心。随机分为3组,每组8只。对照组:标准饲料;实验Ⅰ组:标准饲料+胆固醇喂养4周;实验Ⅱ组:标准饲料+胆固醇喂养8周。胆固醇(长沙生物化学制药厂,纯度98%,磷脂含量0.1%)每日1.0 g混入饲料中,喂6 d后停饲1 d。
实验开始前测定兔血清总胆固醇含量,以血清胆固醇<3 mmol/L为正常标准,血清胆固醇>3 mmol/L即弃去该兔。血清总胆固醇>10 mmol/L定为高胆固醇血症标准[4]。
二、检测指标
(一)Oddi括约肌压力测定用Cooker公司生产的5F血管造影导管自行改制成测压导管。导管长度90 cm,外径1.6 mm。将尖端封闭,距尖端10 mm处开一直径0.4 mm侧孔。测压仪器:RM-6200C四导生理记录仪(成都仪器厂),PT-16M压力传感器(上海高联传感技术公司),WZS-50微量灌注泵(浙江医科大学医学仪器厂)。灌注水流速度恒定于0.25 mL/min,测压系统测压前校准。
术前禁食14~18 h,氯氨酮(陕西省医药研究所) 30 mg/kg肌注麻醉,以后每0.5 h追加半量。取上腹正中切口开腹,切开十二脂肠,经十二指肠乳头将测压导管逆行插入胆总管。描记胆总管压力曲线,稳定5 min后开始测压。每次向外牵拉约2.0 mm直至测压孔完全进入十二指肠,分段描记压力曲线,记录SO各处基础压,收缩波幅。压力曲线以十二指肠压力“零”基线。经导管注入30%泛影葡胺,待肝外胆管全部显影后以25幅/S电影拍摄SO段排胆时的形态特征。
(二)一氧化氮合酶(nitric oxide synthase, NOS)的NADPH染色及图像分析每组随机取3例做NADPH染色。测压后立即切取SO组织,置于4%多聚甲醛PBS溶液中,在4℃下放置16~18 h,随即在10%蔗糖溶液中低温保存。恒冷箱(-25℃)冰冻切片(30μm厚),封固于载玻片上,然后按下列顺序染色:1 mmol/L NADPH(还原型),0.2 mmol/L硝基四唑氮蓝染色,0.1 mmol/L Tris HCl(pH7.2),0.2% Triton X-100在37℃下孵育30 min[5],切片。采用同济医科大学清平影像工程公司多媒体彩色病理图文分析系统进行分析。每张切片随机在平滑肌部位确定4处做图像分析。亮度调节至平均光密度为100,存入图像,域分割,自动测量单位面积中的NADPH的总数量、总面积、平均玄长和平均空间距,逐一储存数据并进行统计学分析。
(三)新西兰兔于喂胆固醇前、中和术晨取耳静脉血测定血清总胆固醇含量。
(四)SO测压结束后切下胆囊,测量胆汁量,作为禁食状态下的胆囊容积。
(五)组织学检查每组随机选取2例做光镜观察的标本用10%福尔马林固定,HE染色。3例透射电镜观察标本制作:测压后立即取下SO组织,用锋利刀片切成1 mm小块,迅速浸入3%戊二醛溶液中,此过程在1 min内完成。在4℃下预固定2~4 h。按常规进行固定、脱水、包埋、超薄切片,铅/铀双染,JEM-2000EX透射电镜观察。
三、统计方法
实验数据用±s表示,组间采用t检验进行统计处理。
结果
一、对照组及实验Ⅰ组、 Ⅱ组喂饲前血清总胆固醇均正常(<3 mmol/L),各组间均无明显差别。喂饲胆固醇后,实验Ⅰ组、Ⅱ组血清胆固醇分别为(30.61±3.32) mmol/L和(29.03±2.59)mmol/L,明显高于对照组及喂饲胆固醇前实验组血清胆固醇含量(P<0.01),实验Ⅰ组、Ⅱ组间无明显差别(P>0.05)。
