超声心动图无创性估测左室舒张压——与心导管相关性的研

摘要目的:应用经胸超声心动图技术估测左心室舒张压。方法:对68例病人通过二维及脉冲多普勒记录二尖瓣、肺静脉血流及左心房参数,并与心导管检查相结合,分析超声指标与左心室舒张压的相关性。结果:(1)肺静脉参数比二尖瓣参数与左室舒张压更具相关性;(2)左室舒张末压(left ventricular end diastolic pressure, LVEDP)明显升高时,肺静脉逆流峰速(PA)及PA的速度时间积分(PAVTI)增高,PA的持续时间(PAd)延长,肺静脉与二尖瓣A峰时限(PAd-Ad)延长,左房径(left atrium diameter, LAd)增大,左房射血分数降低;(3)PAd-Ad与LVEDP及LVa(左房收缩时左室压上升值)的相关系数分别为r=0.52, r=0.48, P<0.001;(4)多元回归分析示LVEDP与PAd-Ad, LAd, E/A, PAVTI及S Fract 相关,以此建立判别函数对LVEDP明显升高进行预测的敏感性是72%,特异性是78%。结论:综合肺静脉、二尖瓣血流及左房参数,可提高预测左心室舒张压的可靠性。

A correlation study between transthoracic echocardiography

and cardiac catheterization in the measurement

of left ventricular diastolic pressures

ABSTRACTObjective:To evaluate transthoracic echocardiographic assessment of the left ventricular diastolic pressure (LVDP).Methods:68 patients underwent transthoracic echocardiography and cardiac catheterization. The correlation between echocardiographic parameters, including left atrium diameter (LAd), mitral and pulmonary venous flow (MVF, PVF), and left ventricular diastolic pressure was investigated.Results:(1) PVF correlated better with LVDP than MVF. (2) When LVEDP was markedly elevated, the velocity and velocity-time integral of the pulmonary venous A wave, the difference between the duration of pulmonary venous A wave and mitral A wave (PAd-Ad) all increased as well; meanwhile, left atrium became larger with lower ejection fraction. (3)PAd-Ad had the best correlation with LVEDP and LVa (r=0.52, r=0.48, P<0.001); (4) Mutiple regression analysis revealed PAd-Ad, LAd, E/A, PAVTI and S Fract to be important determinants of LVEDP upon which discriminant equation was established to predict marked elevation of LVEDP, with a sensitivity and a specificity of 72% and 78%, repectively.Conclusion: The combination of pulmonary venous flow, mitral flow and left atrium diameter can improve the reliability in the measurement of LVEDP.

近几年,人们发现许多心脏疾患(包括冠心病)时心脏舒张功能(left ventricular diastolic function, LVDF)的减退早于收缩功能(如左室射血分数left ventricular ejection fraction, LVEF)异常出现。前者发病机制、治疗方案及预后均不同于后者,故正确识别和评价LVDF减退有重要临床意义。超声心动图特别是脉冲多普勒评价LVDF的有效性已得到广泛承认。各项舒张指标与有创的左室舒张压的相关性,国内外虽曾有报道,但结论不尽一致[1]。本文继续作此方面的研究,通过经胸超声指标与心导管的左室舒张压的相关分析,探讨一种无创性估测左室舒张压的方法,为临床诊治提供依据。

1材料与方法

1.1对象

68例病人,男性58例,女性10例;年龄32~74岁,平均59岁;检查时均为窦性心律,心率43~90 次/分,平均69次/分;无二尖瓣、主动脉瓣狭窄与返流、先天性心脏病及肥厚型心肌病。根据冠状动脉(冠脉)造影结果,51例诊为冠心病,其余17例中,有高血压及高血压病史者7例,10例病人只有胸闷或胸疼症状,冠脉造影及临床检查未发现异常。

1.2超声心动图检查及测量指标

应用HP SONOS 100CF超声诊断仪。探头频率2.5 MHz,病人取左侧卧位,连接肢导心电图,平静呼吸,在胸骨旁左室长轴记录左室舒张末内径、收缩末内径和左房最大前后径(left atrial diameter, LAd);在心尖四腔平面根据心电图对应时相分别测量左房最小面积和上下径、左房最大面积和上下径。将多普勒取样容积分别置于二尖瓣瓣尖及右上肺静脉入口0.5~1.0cm处,适当调整探头方向和取样部位以获得最佳二尖瓣及肺静脉血流频谱。取样容积3 mm,记录速度50mm.s-1。连续测3个心动周期求均值。测量指标:(1)二尖瓣舒张早期最大峰速(E),持续时间(Ed),速度时间积分(E wave velocity-time integral, EVTI);二尖瓣舒张晚期最大峰速(A),持续时间(Ad)及速度时间积分(A wave velocity-time integral, AVTI)并计算E/A, EVTI /AVTI。(2)肺静脉血流收缩期最大峰速(S),持续时间(Sd)及速度时间积分(systolic velocity-time integral, SVTI),舒张早期最大峰速(D)持续时间(Dd)及速度时间积分(diastolic velocity-time integral, DVTI);房缩期反向最大峰速(PA)持续时间(PAd)及速度时间积分(PAVTI)并计算S Fract[=SVTI/(SVTI+DVTI)]及PAd-Ad(图1)。(3)计算左房最大容积LAmax和左房最小容积LAmin及LAEF[=(LAmax±LAmin)]。

1.3心导管检查及测量指标

病人在超声检查后24 h内行左室造影及冠状动脉造影,并在左室造影前应用6 F猪尾导管描记左室压力曲线,以左腋前线第四肋间为零点。测量指标包括左室舒张末压(LVEDP)、左房收缩前左室压力(PreA)及左房收缩时左室压力上升值(LVa),取5~10个心动周期求其均值。

2结果

2.1不同左室压力组之间各项指标的比较(表1)

LVEDP 明显升高组(≥20 mmHg, 1 mmHg=0.133 kPa)与正常或轻度升高组(<20 mmHg)相比较,PAd延长,PAd-Ad增加,PA及PAVTI 均增加,S Fract降低,LAd及LAmin增大,LAEF降低。

PreA升高组(≥10 mmHg)与正常组比较,PAd亦延长,PAd-Ad亦增加, PA及PAVTI亦增加,S Fract降低。

LVa明显升高组(≥10 mmHg)与正常或轻度升高组比较,Ad缩短,PAd-Ad延长,EVTI/AVTI增加,LAmax及LAmin增大。以上各组差别均有显著性。

2.2各项LVEDP与超声指标分别进行单因素直线相关分析(表2)

LVEDP与PAd-Ad相关性最好(r=0.52, P<0.001),与E/A、EVTI/AVTI、PAd,PA 与PAVTI、LAd、LAmax、LAmin均呈正相关,与A、Ad、AVTI、S Fract、LAEF均呈负相关。

左房收缩前左室压(PreA)与S Fract相关性最好(r=±0.40, P<0.01),与E/A、EVTI/AVTI、PAd及PAd-Ad、LAd正相关,与A呈负相关。
